    I think the process is as follows but it may have changed in the last few months.
    1. Get an Istikdam family visa form - available outside the ministry there is a place you can collect from 7:30am - noon
    2. fill the istikdam family visa form and company letter and do COC.
    3. Take those two forms and your passport copy, sister all to istikdam, iqama etc.. You will immediately know if they approved or not if approved they will give that approval form.
    4. then you need to fill the online mofa form But don't do COC
    5. go to MOFA with the online application, and the istikdam approval form, if all information ok they will accept and the online form status will change to form received.
    5. After some days your visa will be online. INSHALLAH
    Don't forget to make dua and ask Allah to help you
